CVS has been around since two brothers started the first store in 1963 in Lowell Massachusetts. By 1964 it had already grown to 17 stores in the area.

Many people don’t know the “CVS” actually stands for “Consumer Value Stores”, encapsulating the store’s mission in these three words.

I always used to think it didn’t stand for anything, however the three letters are core to the company’s main goal of providing value to its customers.

By 1974 the store had already achieved $100 million in yearly sales and was still growing at a rapid pace. By 1985 it had skyrocketed to $1 billion in yearly sales. More recently in 2011, its annual revenue has reached over $100 billion.

CVS offers three entry methods for their survey.

How To Enter The CVS Survey Online

  1. Make a purchase at a CVS
  2. Save your receipt
  3. Go online to
  4. Then enter your 17 digit survey number you can find on your receipt
  5. Answer all survey questions, and you will be automatically entered into the monthly sweepstakes drawing with a chance to win $1,000

How To Enter By Phone

  1. Make a purchase at a CVS
  2. Use the phone number on your receipt to call-in your survey entry

How To Enter By Mail

As no purchase is required for entry into the sweepstakes, you can do a mail in entry.

  1. Gather a 3″X5″ piece of paper and write on it your name, mailing address and phone number.

Entry For Online CVS Purchases – Did you make an online CVS purchase?

If you made a CVS purchase online, and not in-store, some packages will include a 10 digit survey code you can use to enter the survey at this web address, which is different than the web address you’ll use to enter the survey if you make an in store purchase. Contest Dates

  • Every person is limited to ONE entry per month whether they enter online, by phone or mail
  • NO PURCHASE necessary to enter, may enter using receipt/online survey, by calling phone number on receipt, or by mail-in entry

  1. my receipt id # only has 16 numbers. 5471 8153 6231 2134. the survey asks for 17 numbers. the survey will not work for me

    1. Cannot enter, no place to enter number, choice of language does not work. I have 7 survey numbers given by your pharmacy over the last 30 days, yet IF I COULD ENTER SOMEWARE
      there appears to be only 1 entry per month. Is there a point to all of this ????????????????

        I found your store in ‘target’ , although I like it to be there, within, compared to the target store your section seemed old fashioned , targets display are more modern! So consider being within a more up to date display yourselves – not the same though as target. Perhaps a new display would liven up your section.

  2. all the people at the Hammond La. CVS could not B more helpful,nicer, then they are , if I have any questions Sal or any one of the other people will gladly answer it. the coupons R great the service is great Thank them so much.

  3. My daughter is on vacation from Ga to Las Vegas. We did not have time to get her prescription filled. The pharmacist Chelsea at store 4119 went above and beyond to help. The pharmacist Chelsea was able to help transfer my daughter’s prescription so she could breathe and stop her throat pain. I appreciate her help so much I was stressed from my daughter feeling I’ll in another state. Please commend Chelsea in the Augusta Ga CVS on Peach Orchard rd.

  4. Very disappointed a simple entry can’t be made . maybe you don’t want winners? Only time I was able to get in the system refused to accept my number. Trying later the system would not go to the necessary screen. It went to a health screen that said was permanently closed. Waste of time.

  5. It says the page doesn’t exist. I wanted to fill it out because when I got home, I realized that my Rx had another person’s name on it. I had to go back to have it corrected the next day. Alarming!

  6. I have been a customer of CVS for many years and enjoy its many advantages, however the long cash register discount receipts are very annoying and wasting paper. Also you have so many items on sale that it negates most of the discount mailings and further aggravates the situation. I will start buying part of my purchases from Walgreens and/or Rite-Aid. You were supposed to eliminate this system a long time ago!
    Many customers in the store also are irritated with your merchandising.

  7. you have made improvements from changing two lanes to the counter into one lane, however, the counter is very long and its hard keeping your eyes on all the available salespersons behind the counter. Take it one step forward and place a red and green light system at each register so that customers can see at a glance which salesperson is available. Salespersons usually have to wave or shout to a customer that they are available. Its simple but it works using the light system.

  9. There is no place to enter my 17 digit number. Just want to say that you have awesome people working at the pharmacy on 51st avenue and Olive in Glendale, Arizona. These pharmacy techs are so very busy and they work really hard at getting our prescriptions out. For as busy as they are their attitudes are so awesome and helpful. Your store clerks are very helpful and they too help the pharmacy with smiles when called upon. Thank you

  11. At CVS on Keywest highway, MD. Nathan served me and did an excellent Job. He was informative and polite.
    He has a energetic personality, and that is hard for many people when standing behind a counter all day trying
    to help people that may not have very good attitudes at times. Thank you Nathan

  12. How are survey winners contacted? I did get an email from but didn’t open it because I thought it might be a scam or virus. I checked on google to investigate but when I returned to my mailbox the message had disappeared.
    I think winners should be contacted via regular postal mail so they don’t have to worry about the authenticity of the source.

    1. Likely it wouldn’t be from a gmail address since they would likely send from or email addresses. Try contacting the official cvs site contact form to find out for sure, sorry I can’t answer completely!
